How to Register

Registering as a new patient is as simple as the three steps below:

First of all you need to make sure you live within our catchment area, so take a look at our practice area map.


To make your life easier why not print out and fill in these forms which you will otherwise need to do when you get to the surgery: Medical History Form and Family Doctor Registration Form. If you are unable to print them please call us on 01626 332182 and will will send them. Please note on the form your preference of Doctor you wish to register with: Devon Square Team


We offer and recommend a healthcheck to every new patient taking approximately 20 minutes. Our friendly receptionists will gladly book one for you. Please come a little early to allow us to register you, and don't forget to bring your completed forms.


New Babies

Parents of new babies are advised to register the child as quickly as possible. Please use the same Family Doctor Registration Form but it is important that the NHS number is included on the form - this can be found in the babies Red Book issued at birth at the hospital.


Temporary Residents

If you are temporarily staying in the area we can register and treat you as a "Temporary Resident". If you are likely to be here for 3 months or more it is probably worth registering and we can have your records transferred.
