Repeat Prescriptions

Requests for repeat prescriptions can be made using the form on the left.

Any repeat prescriptions to be collected from the surgery will be available after 2pm the following day.

If you wish to have your prescription sent to the chemist of your choice for collection, please allow 48 hours before collecting your medication.

Alternatively you can make the request by post and if you include a stamped address envelope, we will be happy to send you your prescription.
